Documentation will be prepared on your behalf by Global Postal
Solutions ('GPS'), a trading name of Ozepost Pty Limited ('OPL'),
a Regulated Agent with the Department of Transport and Regional
Services from the information entered on this SLOI, and on the
Conditions of Carriage on the reverse of your Customer Copy.
OPL at all times is required to follow procedures and instructions
set-out from time to time by the relevant authority/ies and
other parties associated with the freight of your goods.No liability
is provided for any actions and/or delays associated with our
compliance obligations.
Please retain this copy as your reference point and receipt
of shipping.
‘Door to Door’ is provided unless notated otherwise,
however is always subject to the recipient countries service
level to the destination address. If you wish to receive clarification
please contact the recipient post office or entity.
Responsibility for the weight of the item is solely the Shipper's.
Overweight items may be sent at our absolute discretion by a
downgraded service.

lack of information (or inaccurate) can cause delays and/or
confiscation.
Please attach photo ID of yourself for all air shipments, ID
must be identitcal to senders' name and name on credit card.

CONDITIONS OF CARRIAGE
I Declare:
1. I have read and acknowledge the SLOI obligations and the information
contained within ‘For Your Information’ above.
2. That in the event information is sourced that indicates prohibited
or dangerous goods are in the freight, it may be delayed or forfeited.
3. I have read and understand the Terms
and Conditions of carriage on the rear of this form I am signing.
4. That the reference information is a guide only, and it is my
responsibility under all circumstances to meet and observe all customs
requirements, including but not limited to the exclusions, duty,
taxes and charges that may be levied in the country of destination.
5. That unless specified and paid for, items are not covered for
delay, are not traceable, and no liability exists for GPS and OPL
legal obligations as a Regulated Agent and other regulatory authorities
in the country of dispatch, transit or destination.
6. I have read and understood the above and have been provided with
the necessary documentation required to send my items internationally.
